JOB SUMMARY:
The Division of Developmental Disabilities (DDD) Eligibility Case Manager 2 serves members with developmental disabilities by supporting them and their families to lead a healthy, self-sufficient and productive life.
JOB DUTIES:
- Conducts eligibility interviews; reviews eligibility referral/redetermination; tracks redetermination; compiles information regarding eligibility determination/redetermination from a variety of sources in order to make recommendations on DDD eligibility determinations
- Receives referrals, screens referrals and recommends community resources
- Writes comprehensive progress notes, sets-up new member files
- Reviews documentation, makes eligibility determinations and researches disability diagnoses
- Corresponds with families on initial applications, denial of service notices, attends trainings and meetings
KNOWLEDGE, SKILLS AND ABILITIES (KSAs):
Knowledge of:
- Federal and State statues and agency policies relating to the care and assistance for individuals with developmental disabilities
- Medical and behavioral health issues and terminology
- Health Insurance Portability and Accountability Act (HIPAA)
Skill in:
- Oral and written communication
- Interpersonal relations as applied to contacts with members, representatives of other governmental and community organizations and the public
- Using various computer software (Microsoft Word, Excel, etc.)
Ability to:
- Work independently and within diverse work teams
- Organize and manage eligibility case load
- Document files and information within the required/mandated timelines
SELECTIVE PREFERENCE(S):
- Candidate must possess two (2) years of professional experience coordinating social service activities (e.g., ISP’s, client referral for services, community services, etc.); OR one (1) year as a Human Service Specialist 1 in Arizona State Service; OR a Bachelor’s or Master’s degree from an accredited college or university
PRE-EMPLOYMENT REQUIREMENTS:
- Level One Fingerprint Clearance Card
- Subject to a search of both the Child Protective Services Central Registry and Adult Protective Services
- Valid Driver’s License
- Background and Reference Checks
