JOB SUMMARY: 

This position serves children with developmental disabilities and their families. The purpose of early intervention is to support the family in their relationship with their child with a delay or disability. The end result of early intervention is to develop a firm foundation, that is convenient and attainable for the family, upon which the family may challenge and nurture development to build a child's independence and well-being in all environments throughout their lifetime. The Early Intervention Case Manager may act as the Lead Worker in the Unit and is responsible for more complex and high-profile cases and serves as back-up to the Supervisor in the lead capacity. 

JOB DUTIES: 

- Eligibility determination for the Division of Developmental Disabilities (DDD) and Arizona Early Intervention Program (AzEIP). 

- Following dual agency (AzEIP and DDD) policy and procedures. 

- Development of the Individualized Family Service Plan (IFSP). 

- Coordination of identified supports and services for families and children of greater complexity. 

- Transition out of early intervention. 

- Driving vehicle to conduct home visits. 

KNOWLEDGE, SKILLS AND ABILITIES (KSAs): 

Knowledge of: 

- Methods, procedures, and techniques used in the development of Individualized Family Service Plans. 

- Federal and State statutes and agency policies and procedures relating to the care and assistance for individuals with developmental disabilities. 

- Health Insurance Portability and Accountability Act (HIPAA) 

- Principles and practices of case management and community resources. 

- Medical and behavioral health issues and terminology. 

- Principles of applied behavioral interventions and child development principles. 

- Social, emotional, health, and/or educational needs of children. 

Skill in: 

- Oral and written communication abilities, and conflict resolution management. 

- Developing, evaluating, and maintaining member service plans. 

- Interviewing, counseling, and interacting with members and family members. 

- Conducting needs assessments, problem identification, and resolution. 

- Interpersonal relations as applied to contacts with clients and representatives of governmental and community organization and the public. 

Ability to: 

- Work independently and within diverse work teams. 

- Organize and manage a case load. 

- Document case files and information within required and/or mandated time lines. 

- Maintain confidentiality. 

- Drive a motor vehicle. 

- Lead a team of Support Coordinators (SC) in the absence of the supervisor. 

SELECTIVE PREFERENCE(S): 

- A Bachelor of Arts (BA) or Bachelor of Science (BS) degree in Early Childhood (EC), EC Special Education, Family Studies, Speech and Language Pathologist (SLP), Physical Therapy (PT) or Occupational Therapist (OT) 

- OR a BA/BS degree in a closely related field with related coursework & approved by AzEIP and 1 year experience working with people with developmental disabilities (DD) 

- OR Division Specialist or Service Coordinator hired in AzEIP system before 7/1/01, documentation of continued satisfactory work in AzEIP system from hire date & 3 years of Service Coordinator experience including 1 year experience working with people with DD required. 

PRE-EMPLOYMENT REQUIREMENTS: 

- The selected employee shall have a valid Level One fingerprint clearance card issued pursuant to Arizona Revised Statute 41-2758.07 in order to work with children and vulnerable adults. 

- Candidates for this position shall be subject to a search of the Child Protective Services Central Registry pursuant to A.R.S. 8-804. 

- In-state travel is required for State business. Employees who drive on state business must complete any required driver training (see Arizona Administrative Code R2-10-207.12.) and have an acceptable driving record for the last 39 months including no DUI, suspensions or revocations and less than 8 points on your license. If an Out of State Driver License was held within the last 39 months, a copy of your MVR (Motor Vehicle Record) is required prior to driving for State business. Employees may be required to use their own transportation as well as maintaining valid motor vehicle insurance and current Arizona vehicle registration; however, mileage will be reimbursed.

Positions in this classification participate in the Arizona State Retirement System (ASRS).
