Community Services Intake Coordinator

Summary - In accordance with Assets, Inc. core values,mission and code of conduct and under the supervision of the Community Services Administrative Support Manager, this position performs clerical and intake functions needed to support the intake and service operations of the Community Services Department. Processing and answering a high volume of service inquiries, obtaining client data, indentification, eligibility and insurance information; monitoring, tracking and continue follow-up through the intake process; review and presenttion of intakes to the intake committee; regular coorrespondence with outside entities to obtain needed supplemental ifnormation for eligiblity determination; completion of DLA-20 assessments upon intake; database management, referral and community networking/outreach to broaden the organizations scope of services. This position may require deescalating verbally or physically agressive individuals according to indentified procedures and individual client protocols.
Essential duties and Responsiblities
1. Adheres to and demostrats Assets, inc. core values,mission, policies procedures and compliance plan in the performance of all aspects of this position.
2. Main point of contact for all inquiries specific to services provided by the Community Services Department (i.e., walk-ins, telephone inquires, etc).
3. Conducts the Initial Inquiry screen tool for eligiblity and service determination at the time of initial inquiry.
4. Maintence of the Intake Flowsheet hroughout the intake process to ensure timely follow-up and process flow.
5. Maintenance and distriution of the Supported Living Application.
6. Reviews each supported Living Application upon submittal to ensure it is complete and that all required supplemental infromation needed to support eligiblity criteria and need for services is included (this includes Medicaid eligibility, Medicare,etc).
7. Disseminates the completed Supported Living packet to the iIntake Committee and attends the weekly service coordniation meeting to present new intake and report on requested follow-up from previous intake discussion.
8. Responsible for on-going correspondence and follow-up witht he referral source and other treatment providers when additional information is needed to determine acceptance into services.
9. Provides referral information to appplicants on alternative and/or additional services if necessary (i.e., crisis services, other community providers, community resources, substance use evaluaion, etc.).
10. Schedules and faciliates the intake meeting and orientation to services upon acceptance into Community Services.
11. Completes DLA-20 assesments for new intakes if Waiver funded.
12. Responsible for database entry for new intakes; this will include data entry responsiblities once an EHR is indentified.
13. Responsible for cultivating strategic partnerships through community outreach activities; presentations to organizations who serve similar prpulations to ensure the community is aware of our services and treatment options.
14. As part of a team, ensures effectve and efficient service coordination. This includes linking individuals to services, and advocating for service improvements, and planning for services. Ensures all team members work cooperatively with partner teams.
15. Develops cooperative relationships with community agencies, individual's family members, physicans and all interested agencies and individuals.
16. Identifies and actively participates in required training programs and meetings.
Minimum Qualifications
Education/Experience
Bachelor's Degree in Human Services, Social Work, Business or a related field; and
Two (2) years of direct experience in the field (i.e., case management, planning, etc.); and
Two (2) years intake or customer service experience;
OR equivalent combination of education and experience required.
Must be proficient in using Microsoft Word, Excel and Outlook.
Alaska dirvier's license and insured vehicle required. Must be able to pass a criminal background check under the staate regulations.
